Eligibility Requirements
To qualify for a Kentucky cannabis card, you must:
Be a Kentucky resident.
Provide medical documentation to support your eligibility.
If you are under 18, a parent or legal guardian must provide written consent and act as your caregiver.
Conditions That Qualify
Patients must have at least one of the following:
Any type or stage of cancer
Chronic or debilitating pain
Epilepsy or seizure disorders
Multiple sclerosis, muscle spasms, or spasticity

What You’ll Need
To help our medical marijuana doctor or nurse practitioner evaluate your eligibility, please provide one or more of the following:

Discharge summaries from a hospital visit
Referrals or notes from another medical professional, such as a chiropractor
Current prescription medications or your prescription history from your pharmacy
Proof of prior medical marijuana status in another state
What to Expect and Cost
Your first appointment must be completed in person, as required by law. During this visit, we’ll evaluate your current health conditions, medical history, and any supporting documentation you provide. If you qualify, you’ll receive a written certification form. From there, we’ll handle submitting your application to the Kentucky Office of Medical Cannabis so you can receive your medical marijuana card.
The cost of a medical marijuana evaluation is $200. This fee also applies to renewal appointments, even though renewals can be conveniently completed via telehealth. Kentucky medical marijuana cards are valid for one year, so be sure to schedule your renewal on time to maintain access to your treatment.
